Ambulatory surgery centers (ASCs) reporting quality performance measure ASC-12 (Facility 7-Day Risk-Standardized Hospital Visit Rate after Outpatient Colonoscopy) have until Nov. 18 to review their results for the reporting period Jan. 1, 2018 – Dec. 24, 2019, and July 1, 2020 – Dec. 31, 2020, in their Facility-Specific Report (FSR) mailbox.

It’s important to make sure the information is correct by Nov. 18 before the Centers for Medicare & Medicaid Services (CMS) publicly reports the measure in early 2022.

How to access your report

You can access your FSR mailbox by:

  1. Logging into the MFT Dashboard.
  2. Entering your HCQIS Access Role and Profile (HARP) username and password.
  3. Click on Mail in the left-hand navigation pane and locate your FSR in the Inbox folder.

Your facility will not receive an FSR if:

1. Your facility was not open during the FSR reporting period.

2. Your facility did not have any eligible cases for the measures during the FSR reporting period.

3. Your facility did not have a HARP and MFT account with the appropriate designated roles.

You can use the Claims-Based Measure Lookup Tool to identify if your facility has a FSR available for download. A mock FSR with simulated facility-specific data are available on QualityNet for colonoscopy measure ASC-12.

Submit any questions through the QualityNet Question and Answer Tool.
